Print Summer Program Documents

Below, you will find all of the library's 2019 "A Universe of Stories" Summer Program documents to print yourself! Click on each link to download and then print. (We have plenty forms available at the library. This is just for those wanting to have one step done already when registering for the Summer Reading Challenge.)

Please note:

• Each age group has a registration form that must be turned in before their Reading Challenge may begin. Staff must witness parents signing release form portion of registration forms for their children or teens.
• Reading Challenge Logs may need cut in half after printing as they appear in the below documents. 
• To register for Summer events that require registration, please visit the library during open hours or call us at 419-358-5016.
• Please turn in your registration forms and challenge logs at the library during open hours. None of these documents will be accepted by email or mail.
